At Casa Pacifica, we believe every young person deserves a chance to thrive — not just survive.
Our Transitional Youth Services (TYS) Department offers a lifeline of comprehensive support and services for system-involved or impacted young adults (ages 18-25) who are navigating the complexities of early adulthood and are at risk of housing insecurity.
Guided by the evidence-based Transition to Independence Process (TIP) model, our dedicated and TIP-trained team is committed to a youth-driven and strengths-based approach.
We understand the unique challenges young adults face and provide a continuum of support to promote healing, foster independence and ensure a successful transition to long lasting self-sufficiency.

The TYS Department achieves its mission through three interconnected service areas: Stepping Stones, UpLIFT and Outpatient Therapy.
TYS Offers:
- Individualized case management
- Mental health and wellness support
- Facilitated access to essential health care services
- Practical life skills and financial literacy training
- Support for employment and educational advancement
- Opportunities for college or trade school scholarships
- Parenting classes and peer-to-peer mentorship

Stepping Stones Transitional Housing Program provides immediate, stable housing for young adults transitioning to independence, using a Housing First approach. It offers voluntary, personalized support guided by the Transition to Independence Process (TIP) to help youth build lasting self-sufficiency.
UpLIFT (Unleashing Learning, Inspiring Futures, Fostering Transformation), launched in 2024, offers wrap-around services for transitional age youth, including life skills training, case management, and peer mentorship.


Outpatient Therapy from Casa Pacifica focuses on youth mental health, offering therapy, medication support, and personalized treatment plans to promote emotional wellness and healing.
Who is Eligible?
Young adults ages 18 to 25 who are former foster youth, probation or system-involved youth, or youth who are homeless or at risk of homelessness.
How Do We Measure Success?
We Don’t Just Provide Services – We Deliver Results.
We measure our success by the goals our young adults achieve:
- 100% will open and maintain a bank account
- 100% will avoid involvement with the criminal justice system
- 100% will experience no crisis episodes
- 95% will establish at least one permanent connection
- 90% will be in school, job training, or working 80+ hours/month
