Transitional Housing Plus Foster Care (THP+FC)

 

Casa Pacifica Centers for Children and Families is licensed by the State of California to provide housing and services (THP+FC) to non-minor dependents (NMDs) between the ages of 18 to 21 years. Participants have two different living environment options: Shire apartments (remote site) and Stepping Stone Houses (single site). NMDs receive the support of supervising staff and mentors within the community to accomplish their goals. Casa Pacifica’s Transitional Youth Services (formerly known as CITY), works closely with county social workers, probation officers and SELPA coordinators to ensure participants living in the THP+FC are supported in practicing the skills necessary to successfully transition to independent living.

Casa Pacifica provides high quality living environments combined with supportive services that aid participants in their development as they improve their ability to successfully support themselves and become vital members of their communities. The focus of the program is to ensure that participants move forward on five key life domains: employment, education, living situation, community effectiveness, and personal effectiveness. More specifically, Transitional Youth Services (TYS) staff will support participants in adjusting to and stabilizing in their new and more independent living situations, completing their high school education and/or moving on to higher education or vocational training, developing their job skills and employment related goals, becoming involved in their community, and developing and maintaining healthy relationships.

Transitional Youth Services (TYS) offers the following services:
• Intensive case management
• Individual therapy
• In-vivo life coaching (in the moment)
• Youth advocate support
• Peer-to-peer support
• Kindle Family Finding

After the initial life-planning session, a TYS team member is assigned to the youth to assist with progress towards his/her life plan goals. Additional services, including therapy, are available to youth to support them in the attainment of those goals. Team meetings with the youth and TYS staff occur monthly or more often as needed to monitor individual progress.

To apply for THP+FC contact your current Social Worker or Probation Officer to request an application and tour of the program.
