Casa Pacifica is here for our community’s most vulnerable children and youth! Our Enhanced Care Management (ECM) program supports eligible youth facing complex mental and physical health challenges by coordinating care, connecting families to resources, and assisting with practical needs like housing and transportation.

Our Ventura County ECM team works directly with families and youth to:

  • Connect with doctors, dentists, and therapists
  • Coordinate care across multiple systems
  • Link to local programs and community resources
  • Assist with housing, transportation, and other support services

Who is Eligible:

Families and youth in Ventura County receiving health services through Medi-Cal Kaiser or Medi-Cal Gold Coast. Eligible participants include:

  • Children, teens, or young adults through 20 years old involved in the foster care system
  • Children, teens, or young adults enrolled in CCS/CCS Whole Child Model
  • Children and families at risk of or experiencing homelessness
  • Children with mental health or substance use diagnoses
  • Children transitioning from hospitalization
  • Pregnant or postpartum individuals
  • Individuals involved with the justice system

Our Santa Barbara County ECM team provides the same comprehensive support tailored to the needs of local families. Services are provided in the home, via telehealth and in the community, wherever and whenever is most convenient for the family, and include:

  • Connecting with healthcare providers and therapists
  • Coordinating care across multiple systems
  • Linking to community programs and resources
  • Supporting housing, transportation, and other essential needs

Families and youth in Santa Barbara County receiving health services through Cen Cal. Eligible participants include:

  • Children, teens, or young adults through 20 years old involved in the foster care system
  • Children and families at risk of or experiencing homelessness
  • Children with mental health or substance use diagnoses
  • Children transitioning from hospitalization
  • Pregnant or postpartum individuals
