UpLIFT (Unleashing Learning, Inspiring Futures, Fostering Transformation) Established in 2024, UpLIFT provides comprehensive wrap-around services designed to guide, support, and empower transitional age youth. Through personalized goal-setting, intensive case management, valuable life skills training and supportive peer mentorship, UpLIFT supports young adults in 5 key areas of their lives:

- Personal Effectives and Well-Being
Through a Transitional Independent Living Plan tailored to their strengths and aspirations, our youth gain the skills and confidence to create lasting stability.

Who is Eligible?
